All returning students are required to have a physical exam done every year before beginning the new school year. Please make sure you take the time and make an appointment with your child's doctor. Click here for the necessary form to be filled out by the doctor. Any new vaccinations should also be noted on the form.

All new students need a completed set of medical records, which includes a current physical exam, before attending classes. See below for appropriate forms.
